Technical Assistance:
Discussed with Center Supervisor that children's physical must be maintained in each child's record by 30 days of enrollment.

Comments:
An unannounced monitoring inspection was conducted from 8:10 am-9:30. There were a total of 41 children with 4 staff members present. Children were observed engaged in group activities, sand art and games in the gym. A selection of staff trainings and children records, medications and other documents were reviewed. Standard #: 22VAC40-185-420-E-3
Description: Based on record review and staff interview, staff have not requested at least annually parent confirmation that the required information in the child's record is up to date. Evidence: Two out of Five children records did not have documentation of an annual update. Child #1 enrolled 8/23/17 and child #2 enrolled 10/1/15.

Plan of Correction: I will have all the parents sign that the records have been updated.
