Comments:
An unannounced monitoring inspection was conducted from 8:20am-10:00am. Upon arrival there were 12 children in care with 2 staff present. A sample of 3 children's records were reviewed. The center did not have any medication at the time of inspection. The children did a hands-on activity using marshmallows and raw noodles and played the "Guess Who Game." After the children cleaned-up they gathered their book bags. School staff came and retrieved the preschool children. The school-age children went to the cafeteria and then to class.

Standard #: 22VAC40-185-60-A
Description: Based on a record review the center did not ensure all required information was kept in each child's record. Evidence: Child #2's (dob 06//24/12) enrollment form dated 09/05/17 listed the child's father. The enrollment form dated 09/17/18 did not list the father under parent/guardians, however the father's name was listed under "Person(s) NOT Authorized to Pick Up Child." the center did not have the appropriate paperwork regarding custodial rights to support this.

Plan of Correction: The following corrective action was provided by the program director: The director will contact the child's mother and request custody papers. The director will explain the custodial parent's right and will obtain any other documentation necessary.

Description: Based on a record review the center did not ensure an annual fire inspection was provided. Evidence: The last fire inspection was dated 03/28/17.

Plan of Correction: The following corrective action was provided by the program director: The director was aware that it was expired and plans to call the fire marshall today to schedule an inspection.

Description: Based on observation the center did not ensure annual approval from the health department. Evidence: The health inspection was dated 09/08/17.

Plan of Correction: Based on a record review the center did not ensure an annual fire inspection was provided. The director plans to call the health department today to schedule an inspection.
