A Subsidy Health and Safety inspection was conducted on 09/28/2018 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted from 8:20am-10:00am. Upon arrival there were 12 children in care with 2 staff present. A sample of 3 children's records were reviewed. The center did not have any medication at the time of inspection. The children did a hands-on activity using marshmallows and raw noodles and played the "Guess Who Game." After the children cleaned-up they gathered their book bags. School staff came and retrieved the preschool children. The school-age children went to the cafeteria and then to class. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDSUB-000-006-B-11 Description: Based on a record review the vendor did not ensure there was a written allergy plan for each child with a diagnosed food allergy to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: Child #3 (dob 07/09/14) enrollment form and physical indicate a food allergy requiring medication. There was no medication for the child on-site or a written allergy care plan. Plan of Correction: The following corrective action was provided by the vendor during the exit interview: The mother of the child has indicated to the director that the child has outgrown the allergy. The vendor will contact the parent and gather additional information to determine if an allergy plan is needed and update the record accordingly.
