An unannounced visit was conducted on 8/23/2018 between 2:30pm and 3:45pm to investigate a self-reported incident reported to licensing on 08/16/2018 regarding staff records. Based on the evidence gathered from staff interviews, and record review, there is not a preponderance of evidence to support the allegation; however, a violation not related to the self-reported allegation was found during the course of the investigation. Standard #: 63.2-1720.1-B-3 Description: Based on staff record review, the center did not have documentation that out of state Central Registry check results were obtained for and employee that resided outside the state of Virginia within the last 5 years. Evidence: Staff#1 (DOH: 12/13/2017) documented that they have resided outside the state of Virginia in the preceding five years, there is documentation on file that the out of state central registry check was filled out and submitted, but the center did not have documentation that results were obtained. Plan of Correction: From this point forward, when out of state Central Registry check results have been requested and no response is received within 30 days, we will send another request and notify them that we have not received their response.
