A Subsidy Health and Safety inspection was conducted on 09/25/2018 to review supplemental health and safety requirements. Today an unannounced monitoring inspection was conducted between the hours of 3:00pm and 5:30pm. I observed 42 children directly supervised by 3 staff. Ratios and supervision were in compliance. Children were observed participating in a fire drill, doing homework, reading books, playing with small manipulative toys, and participating in group play in the gym. Staff were actively engaged with the children and assisting them with tasks as needed. The physical plant, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. The cafeteria was found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. Required staff training documents were on-site. Emergency preparedness supplies in order. Required documents posted. Questions about this inspection may be directed to: email@example.com. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-006-B-11 Description: Based on record review and staff interview, the vendor did not have a written allergy care plan for each child diagnosed with a food allergy, to include instructions from a physician regarding the steps to be taken in the event of a suspected or confirmed allergy reaction. Evidence: Child#1, #2, and #3 have diagnosed food allergies, neither one of these children have an allergy care plan from the physician on-site. Plan of Correction: I will print the form and hand it in to the parents to have it completed by the physician.
