Technical Assistance:
Discussed with center director background checks, maintenance of equipment used in the center, diapering.

Comments:
An unannounced monitoring inspection was conducted from 9:50am to 12:30pm. There were 61 children in care of 12 staff members. The children were observed playing in centers including table top manipulatives, coloring and bingo. Diaper changing was observed with the infant and toddler groups. 5 children and staff records were reviewed. Medication and medication records were reviewed. Documents related to auto insurance and liability insurance were also reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, center did not obtain immunization records for children in care before the first day of enrollment. Evidence: Child # 2's first day of enrollment was 6/26/17. The center did not obtain immunization records before Child # 2 started at the center. The immunization records were not in Child # 2's record.

Plan of Correction: We will get the immunization records by tomorrow.

Standard #: 22VAC40-185-140-A
Description: Based on record review, center did not obtain documentation of a physical exam for children in care. Evidence: Child # 2's record did not have documentation of a physical exam. The physical exam was not obtained by the center within 30 days of enrollment.

Plan of Correction: We will get the physical records by tomorrow.

Standard #: 22VAC40-185-60-A
Description: Based on record review, center did not maintain complete records for children in care. Evidence: Child # 1's first day of enrollment was 9/18/17. Center did not view proof of birth within 7 days of the child's first day of enrollment.

Plan of Correction: I will have the parent bring in the proof of birth.

Standard #: 63.2-1720.1-B-3
Description: Based on record review and staff interview, center did not obtain an out of of state central registry for an employee that has resided outside of Virginia in the past five years. Evidence: 1. Staff # 1's record indicated that she resides outside of Virginia. Staff # 1's hire date is listed as 12/3/03. 2. Staff # 2 stated that a request for an out of state central registry for Staff # 1's was mailed in March or April of 2018. There was no documentation on site to show that the request was sent. 3. Results for an out of state central registry were not obtained for Staff # 1.

Plan of Correction: We will redo the out of state central registry for the staff person.
