A Subsidy Health and Safety inspection was conducted on September 12, 2018, between the hours of 3:40 p.m. and 5:20 p.m., to review the supplemental health and safety requirements. Forty-three (43) students were enrolled and thirty-seven (37) students were in the care of five staff members during the on site inspection. The facility was observed to be clean and supplied with a variety of age/stage appropriate play and craft materials. Program observation was completed. Activities observed/provided included: arrival of students; handwashing procedures; snack (yogurt or cheese stick and cheese crackers); gym; homework or reading time; table/floor activities - painting, blocks, Legos, books etc. and pick-up/departure of students. Follow-up was completed on September 21, 2018, by appointment. Six staff records and five student records were reviewed and/or updated. A licensing inspection was also conducted on these dates; the two inspections combined covered all of the subsidy health and safety requirements. Only the subsidy requirements that were not covered in the licensing standards were reviewed during this supplemental inspection. One violation was cited during the supplemental subsidy health and safety inspection and has been identified on the accompanying Violation Notice. Please complete the "Plan of Correction" and "Date to be Corrected" sections on the Violation Notice, sign/date all inspection documents and return completed, signed copies this week. Standard #: VENDSUB-000-005-B Description: Based on observation and discussion with staff on September 12, 2018, the program failed to maintain a written record of daily attendance that documented the arrival and departure of each child in care, as it occurred, for each group of children. Evidence: 1. When the students separate into groups (classroom and gym), there is no documentation of attendance (arrival/departure) for each group of students. 2. Staff #1 confirmed the current process for maintaining student attendance (by program and not by grouping of students). Plan of Correction: Separate attendance sheets have been created so that whenever a group leaves the afterschool room, an accurate tally and roster is with each group.
