An announced inspection was made to Tri-County Community Action Agency located in South Boston, Virginia on Thursday, September 06, 2018 from 1:00 p. m. to 3:25 p. m. The purpose of today's visit was to review staff records. Three staff records and four board officer records were reviewed today. If you have any questions about this inspection, please contact the licensing inspector, Roshon Casey, at (804) 662-9760. ***PROVIDER*** Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Standard #: 22VAC40-191-40-D-1-A Description: Based on a review of records and staff interview on 09/06/2018 the center did not ensure that a new person designated as an agent had a criminal history record check before the end of 30 days after the change. EVIDENCE: 1. The record of Board Officer #2, who according to staff #4 has been board secretary since 11/14/2017, did not contain a criminal history record check. 2. The search of central registry in the record of Board Officer #2, who according to staff #4 has been board secretary since 11/14/2017, is dated 08/23/2018. 3. The sworn statement in the record of Board Officer #2, who according to staff #4 has been board secretary since 11/14/2017, is dated 07/12/2018. Plan of Correction: Not available online. Standard #: 63.2-1720.1-B-2 Description: Based on record review and staff interview on 09/06/2018, the center did not ensure that the results of a fingerprint based national criminal history check were obtained prior to the date of hire for new board officers.
