Technical Assistance:

The licensing inspector conducted an unannounced monitoring inspection Thursday September 27, 2018 from10:35am to 2:25pm. The children were observed in "open time". During open time the children are self directed and choose which learning classroom they would like to work in. They were observed shooting pom pom balls into a hoop in the gross motor room, playing pirates and dress up, working with wood in the carpentry room and making skeletons in the art room. They were also observed making roads and rivers outside on the playground. All classrooms and playgrounds were inspected today. Ten children?s records and ten employee records were reviewed during this inspection. Medication is being administered and was reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 09/26/2018 Shelter-in-place drills: 04/30/2018 & 09/17/2018 Today, the following child to staff ratios were observed: Pink Room -11:2 Purple Room -7:2 Yellow Room -10:2 Red Room-12:2 Blue Room -12:2 Rainbow Room -14:2 Orange Room-not present Polka Dot Room-11:2 Green Room -12:2 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-160-A
Description: Based on a review of staff records on 09/27/2018, the center did not ensure that one of ten records reviewed contained documentation of a negative tuberculosis (tb) screening no later than 21 days after employment. EVIDENCE: The record of Staff #6, who started at the center on 07/01/2016, does not contain documentation of a tb screening.

Plan of Correction: Sure that staff had tb screening done, just can't locate documentation. Will contact staff and if it hasn't been done then will have her go tomorrow.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records on 09/27/2018, the center did not ensure that one of ten records reviewed contained the address of two designated people to call in an emergency if a parent cannot be reached. EVIDENCE: The emergency contacts listed in the record of Child #5 did not have an address listed.

Plan of Correction: Will have parents provide missing information by Tuesday.

Standard #: 22VAC40-185-280-B
Description: Based on observation on 09/27/2018, the center did not ensure that hazardous substances such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children. EVIDENCE: Containers of paint primer, spray starch and cleaning products were observed in unlocked cabinets in three classrooms. The products were out of the reach of the children in care.

Plan of Correction: Cabinets will be locked tomorrow.
