Technical Assistance:

The licensing inspectors conducted an unannounced monitoring inspection Wednesday, September 26, 2018 from 1:20pm to 4:10pm. The children were observed resting on cots and mats. The infants were observed getting a diaper change, sleeping and playing on the floor with a teacher. The school age class was observed arriving to the center after school. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Five children?s records and five employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 8/27/2018 Shelter-in-place drills: 7/24/2018 & 09/17/2018 Last fire inspection: 7/18/2018 Last health inspection: 9/21/2018 Total enrollment today: 36 Today, the following child to staff ratios were observed: Infants ?4:2 Toddlers ?7:1(rest time ratio) Twos ? 8:1 (rest time ratio) Preschool & Pre-K ?9:1 School Age ?8: 1 The violations from the previous inspections were checked for corrections. There was one repeat violation found during today?s inspection. If you have any questions about this inspection, please contact your licensing inspector, Roshon Casey, at (804) 662-9760. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on a review of children's records on 09/26/2018, the center did not ensure that one of the five records reviewed contained the phone number of two designated people to call in an emergency if a parent cannot be reached. EVIDENCE: The record of Child #3 did not contain a phone number for the second emergency contact listed.

Description: Based on observation on 09/26/2018, the center did not ensure that areas of the center were maintained in a safe condition. EVIDENCE: 1. The wood baseboard on the left wall by the playground door in the preschool room is deteriorating and splintered. 2. A coat hook on the wall right wall in the school age room is broken.
