- Technical Assistance:
An unannounced monitoring inspection was conducted on Thursday, September 27, 2018 from 9:45am to 12:50pm to determine the center?s compliance with licensing standards. The center?s director and assistant director assisted the inspector throughout this inspection. The census for today?s inspection consisted of 68 children in care and 20 staff. The School Age children were not present during this inspection. The licensing inspector observed the children playing in centers and interacting with peers and the staff. The children were also observed taking breaks and washing their hands. The staff were observed having positive interactions with the children. The following information was reviewed: daily attendance, required postings, injury reports, and liability insurance policy. The first aid kit and non-medical emergency supplies were inspected and found to be in compliance. The most recent evacuation drill was documented on 08/31/2018. The last two shelter-in-place drills were documented on 04/25/2018 and on 06/27/2018. The last documented health inspection occurred on 05/31/2018. The last documented fire inspection occurred on 02/09/2018. Seven children?s records and seven staff?s records were reviewed. Standard #: 22VAC40-185-270-A Description: Based on observation at the center on 09/27/2018, the center did not ensure all areas and equipment of the center were maintained in a safe and operable condition. EVIDENCE: 1) In the School Age, Pre-K, and Threes playground, the licensing inspector observed a cracked area of plastic with two sharp edges on the playground's plastic border that was a safety concern for the children who would be using the playground. Plan of Correction: Per the Director: A work order has been placed and the plastic border around the playground will be replaced once the weather permits.
Standard #: 22VAC40-185-280-B Description: Based on observation at the center on 09/27/2018, the center did not ensure to keep all hazardous substances such as cleaning materials in a locked place using a safe locking method that prevents access by children. EVIDENCE: 1) The licensing inspector observed bottles of cleaning solution in two unlocked cabinets in the Toddlers and Waddlers' classroom. 2) The licensing inspector observed bottles of cleaning solution in one unlocked cabinet in the Discovery Preschool classroom. Plan of Correction: Corrected during inspection. Per the Director: Staff will be retrained to lock cleaning supplies.
Standard #: 22VAC40-185-290-3 Description: Based on observation at the center on 09/27/2018, the center did not ensure all electrical outlets had protective covers. EVIDENCE: 1) During the inspection, the licensing inspector observed one outlet without a protective cover in the Discovery Preschool classroom. 2) During the inspection, the licensing inspector observed two outlets without a protective cover in the Toddlers' classroom. 3) During the inspection, the licensing inspector observed one outlet without a protective cover in the School Age classroom, which was being used by the Pre-K class on 09/27/2018. Electrical outlets shall have protective covers that are a size that cannot be swallowed by children. Plan of Correction: Corrected during inspection. Per the Director: Staff will be retrained on using protective covers.
Standard #: 22VAC40-185-580-B Description: Based on observation and interview on 09/27/2018, the center did not ensure that safety standards set by the Department of Motor Vehicles were met for one of three vehicles used to transport the children. EVIDENCE: 1) During the inspection, the licensing inspector noticed that one out of three buses used to transport the children did not have a current state vehicle inspection. The required inspection expired June 2018. 2) During interview, the Director stated that the inspection had not been done. Plan of Correction: Per the Director: Staff will be instructed not to use that bus until further notice. The Director will have the bus picked up to be inspected.
