Technical Assistance:
*** Discussion held with the director about monitoring children who fall asleep in places other than their cribs.

Comments:
An unannounced monitoring inspection was conducted today from 1:00pm-2:45pm. There were 18 children directly supervised by 4 staff. The physical plant, 4 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. There were no medications present at the center. Children were observed napping with teacher supervision. Infants were observed playing on the floor with teachers and small toys, eating and napping. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, one of five children's records was incomplete. Evidence: Child 2's record did not contain the names, addresses, and phone numbers of two persons to be contacted in the event of an emergency.

Plan of Correction: We will ask the parents to provide the missing information.

Standard #: 22VAC40-185-270-A
Description: Based on observation, not all of the equipment inside of the center was maintained in a safe and operable condition. Evidence: A cot in the preschool room was observed with a stretched and ripping mesh that can no longer safely support the weight of a child.

Plan of Correction: The cot will be disposed of today.
