Technical Assistance:
Discussed requirement for all staff to have fingerprinting completed prior to October 1, 2018.

Comments:
An unannounced monitoring inspection was completed today. The physical plant, 6 staff files, 5 children's files, staff training, children's activities, medications, emergency procedures, drills and supplies were inspected between 12:30- 2:15pm. The outside areas were not inspected due to the weather conditions. There were 34 children under the supervision of 8 staff. During the inspection, children were observed reading books, resting during naptime and working in centers where they counted beads, worked on puzzles and various other activities. The children bring in their lunches from home each day they are in attendance. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 4 staff certified in MAT (medication administration training). Should you have any questions, please contact me at mary.wizbicki@dss.virginia.gov.

Standard #: 22VAC40-185-160-C
Description: Based on file review, 3 out of 6 staff records do not have a TB test/screening completed within two years from the date of the previous TB test/screening. Evidence 1. Staff #2's most recent TB test/screening was completed on 9/19/16. 2. Staff #3's most recent TB test/screening was completed on 8/22/16. 3. Staff #5's most recent TB test/screening was completed on 11/28/15.

Plan of Correction: These staff will have this completed again.

Standard #: 22VAC40-185-70-A
Description: Based on file review, 2 out of 6 staff records does not have all required information. Evidence 1. Staff #4 and #6 do not have documentation that two or more references as to character and reputation as well as competency were checked before employment began; written information to demonstrate that they individual possesses the orientation training required by the job position.

Plan of Correction: These items will be obtained/completed and kept in their files.

Standard #: 22VAC40-185-280-B
Description: Based on observations, there were hazardous substances in areas accessible to the children in care. Evidence 1. In the middle classroom, there was a bottle of insect repellent and hand sanitizer on top of a shelf, which was within reach of the children in care. 2. In the back classroom, there was a bottle of hand sanitizer on top of a shelf, which was within reach of the children in care.

Plan of Correction: Upon notification, these were immediately moved to an area inaccessible to the children.
