Comments:
An unannounced monitoring inspection was conducted on 9/28/18 from 10:00am - 12:40pm. When the Licensing Inspector arrived there were 54 children ages two years old to five years old in care with eight staff. Children were observed participating in various activities in the classrooms, playing outside, during rest period, and eating lunch. Records were reviewed for six children and six staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of six children's records and interview, it was determined that the licensee did not ensure that they maintain and keep at the center a complete record for each child enrolled that contains all required information. Evidence 1. The record for child #1 did not contain the name, phone number and address for two designated people to call in an emergency if a parent cannot be reached. 2. Staff #6 confirmed that the record for child #1did not contain all of the required information.

Plan of Correction: The facility responded : The parents of child number #1 will be asked to provide two emergency contacts that are not themselves. Parents will be notified that it is a state requirement to provide contact information for two emergency contacts even if they are out of the country/not local individuals. Parent will be required to provide information prior to student returning to school.
