An unannounced Monitoring Inspection was conducted on October 1, 2018 from approximately 4:00 pm until 5:35 pm. Upon entrance there were 35 children and 4 staff present with children. The record review sample consisted of 6 children's records and the required documentation on site for 4 staff. Children were observed playing large group games in the gymnasium and working on homework, coloring and playing board games. Four medications were reviewed. Description: Based on observation and staff interviews, the licensee did not ensure that when centers choose to provide meals or snacks, all the requirements were followed. Centers shall follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Children shall be allowed second helpings of food listed in the USDA pattern. Evidence: During the inspection on October 1, the following was observed as it relates to snack for the schoolage children. 1) Six children, grades 3rd to 5th were observed eating snack and one child requested a helping of yogurt and the center staff indicated they were not allowed to have a second helping. 2) The children ages 5- 11 were served yogurt and grapes as a snack. The grapes measured 1/2 cup and 3/4 cup was required.

Plan of Correction: The center director will print off the USDA guidelines and retrain staff.
