Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted from 9:30 am - 11:45 am. There were 88 children in care being supervised by 13 staff members. The children were observed participating in circle time activities, crafts, and cooperative games. Lunch served today is turkey and cheese sandwich, mixed vegetables, peaches and milk. Ten children's records were reviewed. Six staff records were reviewed. An allergy list was posted for children enrolled in the program with diagnosed food allergies. Allergy care plans were reviewed for some of the children enrolled in the program with diagnosed food allergies. Staff training was reviewed for completion of CPR, First Aid, and Virginia Preservice Training. Standard #: VENDSUB-000-006-B-11
Description: Based on review of the programs posted allergy list it was determined that not all children with diagnosed food allergies had an allergy care plan on file. Evidence: Child A, B, C, D, E, F, G, and H with diagnosed food allergies did not have an allergy care plan on file.

Plan of Correction: Administration will request an allergy care plan from the parents of child A, B, C, D, E, F, G, and H.

Standard #: VENDSUB-000-012-B
Description: Based on review of staff training it was determined that not all staff records contained documentation of completing the Virginia Preservice Training within the first 90 days of employment. Evidence: Staff A (start date: 04/02/2018), Staff B (start date: 05/14/2018), Staff C (start date: 09/05/2011) did not have documentation of completing the Virginia Preservice Training.

Plan of Correction: Staff A, B, and C will complete the Virginia Pre-Service Training.

Standard #: VENDSUB-000-012-E-1
Description: Based on review of training it was determined that not all staff maintained current certification in CPR. Evidence: 1. Staff B with a start date of 05/14/2018 did not have documentation of completing a CPR course. 2. Staff D, E, and F had documentation of CPR which expired 8/31/2018. Staff G had documentation of CPR which expired 7/23/2016.

Plan of Correction: Staff B, D, E, F, and G will enroll in a CPR course.

Standard #: VENDSUB-000-012-E-2
Description: Based on review of training it was determined that not all staff maintained current certification in First Aid. Evidence: 1. Staff B with a start date of 05/14/2018 did not have documentation of completing a First Aid course. 2. Staff D, E, and F had documentation of First Aid which expired 8/31/2018. Staff G had documentation of First Aid which expired 7/23/2016.
