An unannounced monitoring inspection was conducted from 7:30 am - 9:15 am. There were 34 children in care being supervised by 3 staff members. The children were observed participating in a craft, preparing for and eating breakfast. Breakfast served today is turkey sausage bagel, cereal bar, apple slices, and milk. Ten children's records were reviewed. Three staff records were reviewed and in compliance. Twelve medications were reviewed. There are currently two staff members on site who are MAT (Medication Administration Training) certified. Standard #: 22VAC40-185-130-A Description: Based on review of ten children's records it was determined that not all records contained documentation of immunizations before the first day of enrollment. Evidence: Child A (Start Date: 08/13/2018), Child B (Start Date: 08/15/2018), and Child D (Start Date: 08/16/2018) did not have documentation of an immunization record on file. Plan of Correction: Administration will request an up to date immunization record from the parents of Child A, B, and D.
Standard #: 22VAC40-185-140-A Description: Based on review of ten children's records it was determined that not all records contained documentation of a physical within the first 30 days of attendance. Evidence: Child A (Start Date: 08/13/2018), Child B (Start Date: 08/15/2018), Child C (Startd Date: 08/29/2018), and Child D (Start Date: 08/16/2018) did not have documentation of a physical on file. Plan of Correction: Administration will request an updated physical from the parents of Child A, B, C, and D.
Standard #: 22VAC40-185-510-N Description: Based on review of 12 medications it was determined that not all expired medication was returned to parents within 14 days. Evidence: The emergency medication for Child E expired January 2018. The emergency medication for Child F expired August 2018. Plan of Correction: Administration has requested this medication from parents and will request it again, however there is a current back order on the specific emergency medications.
