A Subsidy Health and Safety inspection was conducted on October 2, 2018 from approximately 10:00 AM to 3:30 PM to review supplemental health and safety requirements. The director was present for the inspection. The following staff to child ratios were observed: Infants 3:1, Older 1 year olds and 2 year olds 12:2, 3 year olds 8:1, 4 year olds 6:1. The school age group was not present during the inspection. Staff were observed to be interacting with children appropriately. Today's lunch consisted of mini corn dogs, mixed vegetables, fruit salad, and milk. All areas of the facility including classrooms, bathrooms, hallways, buses and playground were inspected. The facility had the following posted: license, evacuation maps/procedures, daily schedule, menu, and various parent information. Allergy postings and medications were reviewed. Several staff had current First Aid and CPR certifications. The first aid kit and non-medical emergency supplies were inspected and found complete. The emergency preparedness drills were reviewed and found complete. Five children?s records were reviewed. Five staff records were reviewed this day at the administrative office. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on a review of five children's records and an interview, the facility did not ensure that staff has allergy care plans during the transportation of children diagnosed with a food allergy. Evidence: The licensing inspector observed the record for Child #5 indicated the child had a diagnosed food allergy. The provider stated the allergy action plan was not provided to staff during transportation.

Standard #: VENDSUB-000-006-B-11
Description: Based on record review of five children's records and interview, the facility did not ensure children with diagnosed food allergies had a written allergy care plan that included instructions from the physician. Evidence: The licensing inspector observed the record for Child #4 indicated the child had a diagnosed food allergy. The record did not contain an allergy care plan that also included instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. A member of management also reviewed the records and the information could not be located during the inspection.

Plan of Correction: Administration will request the documentation from the child's parents.

Standard #: VENDSUB-000-012-E
Description: Based on review of five staff records, one staff did not have the required first aid and CPR training within the required time frame. Evidence: During the inspection, the licensing inspector observed the record for Staff #3 (hire date 06/25/2018) did not contain documentation of current first aid and CPR certification.

Plan of Correction: Per administration, the training has been completed and the required documentation will be placed in the staff record.
