During the inspection, the licensing representative discussed with staff: - TB results needed for new staff - children's records - progress has been made with staff obtaining lead qualifications; more staff are signed up for CDA training
An unannounced monitoring inspection was conducted today from 11:00am - 3:30pm. A total of 37 children were in the care of 8 staff. Infants were observed during diapering and bottle feeding. Children were observed during outdoor play, handwashing, lunch, and transitions to naptime. A sample of 10 children's records, 5 staff records, emergency drills and other documents were reviewed. Standard #: 22VAC40-185-130-B Description: Based on a sample review of records, the record for one child under the age of 2 years old did not include documentation of additional immunizations once every six months. Evidence: The most recent immunization record for Child #6 [14 months] were obtained 12/11/2017. Plan of Correction: I will request a record of additional immunizations from the parent.
Standard #: 22VAC40-185-270-A Description: Based on observation, areas inside and outside the center were not maintained in clean and safe conditions. Evidence: 1. Areas of chipped and peeling paint were observed on various areas on the walls and/or windows in the toddler classroom, two's classroom, 4's classroom, and school-age classroom. 2. Areas of rust were observed on the wall and around bolts on the commode in the bathroom. 3. A pile of animal feces was observed in a storage closet outdoors near the toddler playground that was accessible to children. Plan of Correction: Work orders will be placed for all maintenance requests.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method. Evidence: The lock used on an upper cabinet in the 4's classroom was not tightly installed. The licensing representative was able to remove cleaning agents by reaching underneath the cabinet. Plan of Correction: The lock was tightened to ensure substances remain inaccessible to children. Corrected during inspection.
Standard #: 22VAC40-185-340-D Description: Based on observations and a review of staff records, a staff member who meets the qualifications of a program leader was not regularly present in 1 out of 6 groupings of children. Evidence: Staff #1 informed the licensing representative that there is currently no staff with program lead qualifications assigned to the Two's classroom. Plan of Correction: We are working to help our staff obtain the required credentials.
Standard #: 22VAC40-185-510-N Description: Based on observation, a medication was not returned to a family within 14 days of the authorization expiring. Evidence: A medication authorization for Child #7 expired 4/21/2018. The medication was observed on site during the inspection. Plan of Correction: I will return the medication to the parent.
Standard #: 22VAC40-185-540-A Description: Based on observations and interview, a first aid kit was not accessible during outdoor play. Evidence: During the inspection, the two's, 4's and 3's classrooms were observed during outdoor play. When asked, staff stated that they did not have first aid kits with them during that time. Plan of Correction: I will remind staff to take first aid kits with them when on the playground.
