Revised Inspection Summary Comments An unannounced complaint investigation began today from 2:30 pm - 4:45 pm regarding Supervision, Programs, Diapering and Special Feeding Needs. Description:	Based on observation during today's inspection, some hazardous cleaning materials were not kept in a locked place using a safe locking method that prevents access. Evidence: In the Toddler 2 room, the cabinet under the diaper table was unlatched, with a cleaning supply accessible at child height while children were sitting nearby. In another room, a cleaning supply was observed hanging on a shelf when diapering or toilet training was not taking place.
Plan of Correction:	Daily walkthroughs will be conducted by management to make sure that chemicals in classrooms are locked up. This gives them coaching opportunities for staff members and additional types of documentation if it is a repeat issue in classsrooms.

Description:	Based on observation during today's inspection, when staff were supervising children, they did not always ensure their care, protection and guidance as required. Evidence: 1. Staff failed to provide care, protection, and guidance on 8/27/18 when a 21-month old child was observed to be outdoors on his own for an undetermined amount of time. At 2:36 p.m., Child #1 was observed in the playground area, between the fence and the door to his classroom. When he turned away from the inspector to get back inside, staff saw him through the door window and opened the door. There were a total of 10 children present with 3 staff at the time. Six children were observed to be sleeping or resting on cots while 3 children were observed coloring and playing with quiet puzzles. Staff had no knowledge that the child was outside. When asked, Staff # 1 reported: "He was supposed to be sitting at the table with the other children." The table was observed to be located near the door to the playground. The temperature in Oakton on 8/27/18 (at 2:30 pm according to the timeanddate/weather.com) was 92 degrees. 2. Additonally, staff failed to provide care, protection and guidance when another child, age 3 years old, needed assistance while but was observed to be without supervision at 2:32 p.m. Child # 2 (age 3 years old) was observed in a hallway bathroom, sitting on the toilet. No staff were nearby for assistance. Child # 2 asked the inspector for "more paper."
Plan of Correction:	a) Retrained all staff on safety and supervision. We discussed the safety factors that take place in both the classrooms and outside the building. The importance of proper name to face counts throughout the day and especially during transitions. Also Teacher and Manager responsibilities with Child Supervision Records. b) Reviewed strategies to provide 'Superior Supervision' in classrooms and on the playground. Discussed the necessity of accurate use of Child Supervision Record, and various transition strategies and scenarios to identify methods of improving transitions, while creating a balance between teachers' responsibility for child supervision, Developmentally Appropriate Practices, and educational programming.

Description:	Based on observation during today's inspection, staff failed to provided sight and sound supervision to children who were under age ten. Evidence: 1. Staff were not within actual sight and sound of Child #1 (21 months old) on 8/27/18 when he was observed alone unsupervised in the playground. He was out of sight and sound for an undetermined amount of time. 2. Staff were not within sight and/or sound supervision of Child # 2 (3 years old) on 8/27/18 when she was observed to be alone in the bathroom at 2:32 pm. No staff were nearby to provide assistance when the child asked for "More paper" while she was sitting on the toilet.
Plan of Correction:	VA licensing supervision standards will be discussed with all staff members and a{n} acknowledgement will be signed that they understand that children under 10 must be within sight and sound at all times.

Description:	Based on observation and staff confirmation during today's inspection, heated bottles were not always tested before serving to infants. Evidence: Staff # 3 was observed today taking a bottle from the warmer, stirring it but not testing it. (breast milk)
Plan of Correction:	Infant teachers will be refreshed on all safe bottle practices. Despite that the bottle was not feed {fed} to the infant we want to make sure they understand the importance of testing temperature prior to feeding.
