Comments:
An unannounced monitoring inspection was conducted on October 5, 2018. At arrival to the facility, there were 24 children with five staff members. The children were observed playing outdoors, having lunch and resting. Infants were observed napping. Licensing Inspector reviewed supervision, programs, activities, equipment, indoor and outdoor areas, children and staff records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, emergency supplies and required postings. Five children's records and five staff records were reviewed. The center is currently not administering medication. The exit interview was conducted with the Assistant Dirrector.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that the center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: During the inspection conducted on October 5, 2018, there was no documentation of immunizations in the record for Child #5 (start date 4/17/18).

Plan of Correction: The child has an appointment scheduled for Monday and documentation will be obtained.

Standard #: 22VAC40-185-140-A
Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that each child shall have a physical examination by or under the direction of a physician: Before the child's attendance or within one month after attendance. Evidence: During the inspection conducted on October 5, 2018, there was no documentation of a physical examination in the record for Child #5 (start date 4/17/18).

Plan of Correction: The child has an appointment scheduled for Monday and documentation will be obtained.

Standard #: 22VAC40-185-60-A
Description: Based on record review, in two of five children's records reviewed, the licensee did not ensure that each child's record was maintained with all required information. Evidence: During the inspection conducted on October 5, 2018, the following information was not documented in each child's record: Child #1- a complete address for one of two persons to contact in the event of an emergency where the parent cannot be reached. Child #5-viewing proof of the child's identity and age

Plan of Correction: The record for Child #1 was corrected during the inspection. The birth certificate for Child #5 has been requested.

Standard #: 22VAC40-185-260-A
Description: Based on review, the licensee did not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection conducted on October 5, 2018, the most recent fire inspection report for the facility was dated 5/17/17.

Plan of Correction: Assistant Director states that the inspection was done this year and they will email the document.

Standard #: 22VAC40-185-320-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that each restroom area provided for children shall be equipped with all necessary supplies. Evidence: During the inspection conducted on October 5, 2018, the girls restroom in the Infant-School Age building did not have soap.

Plan of Correction: The soap was replaced during the inspection.

Standard #: 22VAC40-185-330-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Falls zones shall not include barriers for resilient surfacing. Evidence: During the inspection conducted on October 5, 2018, the following inadequate fall zones were observed on the outdoor playground area and children were playing on the equipment: 1. The height of the wooden swing bay with two round teal/black and one square swing saucer was 6'6" tall and should have at least a 13' fall zone in front and back. The fall zone in front was inadequate and measured approximately 12' in front to the wooden border and back to a picnic table. 2. The height of the wooden swing bay with two swing seats and one green plastic standing swing was 7' tall and should have at least a 14' fall zone in front and back. The fall zone in front was inadequate and measured approximately 12'8" to the fence behind the swings and 12' to the wooden border in front. 3. The height of the wooden swing bay with the outdoor spider web swing and the giant saucer spinner tree swing was 7'4" tall and should have at least a 14'8" fall zone in front and back. The fall zone was inadequate and measured approximately 13' from the picnic table in back and 10'8" to the wooden border in front.

Plan of Correction: We will come out with a six foot extension to increase the fall zone which will cover all three swing bays and add the required depth of mulch in all fall zone areas.

Standard #: 63.2-1720.1-B-2
Description: Based on record review, in one of five staff records reviewed, the licensee did not ensure that staff submit to fingerprinting prior to employment. Evidence: During the inspection conducted on October 5, 2018, the fingerprint print results for Staff #1 were dated 8/7/18 and the hire date as documented in the record was 7/31/18. Orientation was documented as being completed 8/3/18.

Plan of Correction: We will make sure that we do nothing with staff until after the results are back.
