Technical Assistance:
Discussed maintaining last years emergency drills at the school. They were on site today.

Comments:
An unannounced monitoring inspection was conducted from 8:00 am-9:30. There were a total of 45 children with 5 staff members present. Children were observed engaged in group activities and outdoor play time. A selection of staff trainings and children records, medications and other documents were reviewed. Standard #: 22VAC40-185-60-A
Description: Based on record review, each child's record is not complete. Evidence: Child #2 record does not include 2 emergency contact addresses. Child #3 record does not include a physical and immunization record.

Plan of Correction: I will get the addresses from the parent. The school will provide the physical and immunization record by the end of the week.

Standard #: 22VAC40-185-510-G
Description: Based on medication review, one child's medication did not include the original prescription container with the label. Evidence: Child # 1 has a prescription medication, the medication is not maintained in the original container with the prescription label.

Plan of Correction: We will ask the parent for the box with the prescription label.
