Comments:
An unannounced monitoring inspection was conducted from 8:45am - 11:00am. Upon arrival there were 63 children in care with 16 staff present. A sample of 4 staff and 6 children's records were reviewed in addition to 3 medications. There was 1 child in care with a diagnosed food allergy. Children were observed during circle time, being read a book and participating in art. Children were observed on the playground and infants were observed sleeping in cribs, being held and playing on the floor with staff.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a record review the center did not ensure all required information was kept for each staff person. Evidence: Staff #2 and #4 did not have documentation of a complete emergency contact address.

Plan of Correction: The following corrective action was given during the exit interview: The director had staff #2 and #4 fill out their complete addresses in the record.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center, inside and outside were maintained in a safe condition. Evidence: On the playground there was a tricycle that was missing a pedal.

Plan of Correction: The following corrective action was given during the exit interview: The director pulled the trike from the playground during the inspection.

Standard #: 22VAC40-185-430-I
Description: Based on observation the center did not ensure all personal articles were individually assigned. Evidence: In the preschool room there was a tube of chapstick that was not individually assigned. It was not labeled or identified in any way that demonstrated who it belonged to.

Plan of Correction: The following corrective action was given during the exit interview: The director went to check the chapstick. It belongs to a staff member. It was labeled and put in a locked place.
