Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on 8/28/2018 regarding allegations of staffing/supervision and operational responsibilities. The inspection was initiated on 9/5/2018 and completed on 9/6/2018. On 9/6/2018, the inspector was on site from 9:00 AM to 11:30 AM. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there were violations found during the inspection regarding staff records and background checks. See the violation notice for more details. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 15 business days from today. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description: Based on record review, the center did not ensure that the individual staff record reviewed contained all required documentation. Evidence: 1. The record of Staff #1 (start date 5/14/2018) did not contain verification of age, documentation of two or more references, and a job title.

Description: Based on review of one staff record, the center did not discontinue employment after failing to make contact within four business days to follow up on one staff's central registry search request that had not been received within 30 days. Evidence: The record for Staff #1 indicated a hire date of 5/14/2018 and a central registry finding dated 7/12/18. Staff #1's record had documentation that the central registry search request was mailed on 5/24/2018. The record did not contain documentation of any further contact, and the staff has been continuously employed.
