Evidence: 1. The medication authorization for child #3 was signed by the doctor, however the medication authorization was not signed by the parent. 2. The medication authorization for child #9 was signed by the doctor, however the medication authorization was not signed by the parent. 3. The medication authorization for child #10 was signed by the doctor, however the medication authorization was not signed by the parent. 4. Staff #9 (Program Director) reviewed the medication authorizations for child #3, child #9, and child #10, and confirmed that neither parent signed the medication authorization. Plan of Correction: The facility responded: The parents of child #3, child #9, and child #10, will be asked to sign the medication authorizations today (10/4/18). All new medication will not be accepted unless there is written parent authorization for the medication.
