Comments:
A Subsidy Health and Safety inspection was conducted on 10/4/18 from 9:30am to 12:45pm to review supplemental health and safety requirements. At the time of entrance there were 77 children ages seven weeks old to four years old in care with 16 staff. Children were observed participating in various activities in the classrooms, playing outside, eating lunch, and during rest period. Records were reviewed for eight children and eight staff . A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on a review of eight children's records, it was determined that the vendor did not ensure that a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. The record for child #2 contained documentation from a physician that child #2 was allergic to peanuts, wheat, eggs and dairy. However there was no allergy plan present for child #2 at the facility. 2. Staff #9 (Program Director) reviewed the record for child #2 and confirmed that child #2 did have a diagnosed food allergy and that there was no allergy plan available for viewing during the inspection.

Plan of Correction: The vendor responded: The parents of child #2 will be asked to provide a new copy oft he allergy plan. All new children that enrolled and have a diagnosed food allergy will have an allergy plan at the center.
