Comments:
At the time of the tour there were 60 children in care with 9 staff present. A sample of 2 staff and 6 children's records in addition to 2 medications were reviewed. None of the children in care had a diagnosed food allergy. Children were observed during nap. Infants were observed being rocked, playing on the floor and sleeping in cribs. The air conditioning was currently being worked on outside the building. The center made provisions and had fans blowing to keep the rooms cool.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a record review the center did not maintain all required information for each child enrolled. Evidence: Child #1, #3, and #4 did not have complete addresses for the two required emergency contact information.

Plan of Correction: The following corrective action plan was provided by the Assistant Director during the exit interview: The Assistant Director will obtain complete addresses for child #1, #3, #4 today at pick-up.

Standard #: 22VAC40-185-280-B
Description: Based on observation the center failed to ensure hazardous substances were kept in a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to the children. Evidence: In the large middle room there was a cabinet that had the key in the keyhole. Inside the cabinet were three cans of lice prevention spray for bedding.

Plan of Correction: The following corrective action plan was provided by the Assistant Director during the exit interview: The key was removed during the inspection. The Assistant Director will remind staff that the cabinet should be locked and the key should be inaccessible.

Standard #: 22VAC40-185-450-D
Description: Based on observation the center did not ensure when pillows are being used they were covered with pillow cases. Evidence: In the preschool room 2 children were observed napping on pillows that did not have covers.

Plan of Correction: The following corrective action plan was provided by the Assistant Director during the exit interview: The assistant director will remind parents and staff that ALL pillows are required to have covers.
