A Subsidy Health and Safety inspection was conducted on 10/11/18 to review supplemental health and safety requirements. Licensing Inspector arrived at 8:15am and departed at 11:30am to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 56 children in care with 13 staff present. A sample of 5 children and 4 staff records in addition to 2 medications. Children were observed playing in centers and being read a story. Classes were beginning to get ready for lunch. Children were also observed entering the building. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-006-B-11 Description: Based on a record review the center did not ensure each child had a written allergy care plan when diagnosed with a food allergy. Evidence: Child #5 had a diagnosed food allergy indicated on the enrollment form and had an Epi-Pen at the center. Child #5 did not have a written allergy plan to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Plan of Correction: The following corrective action was provided by the director during the exit interview: The director contacted the child's parent during the inspection. Mom stated the child no longer needs the Epi Pen and will obtain documentation to indicate new instructions from the child's physician.
Standard #: VENDSUB-000-006-B-5 Description: Based on a record review and interview with the director the center did not ensure the required information on allergies, including food allergies, intolerances to food, medication, or other substances, and actions to be taken in an emergency situation. Evidence: 1. The center had a list of children's allergies that was posted in each classroom. During the inspection it was discovered that the director wrote the wrong child's name under the picture of her twin, Child #5, who has an allergy to eggs and peanuts that require an Epi-Pen. The Director confirmed that Child #5 had medically diagnosed allergy and Child #4 did not. 2. The center's list of children's allergies didn't differentiate between true medically diagnosed allergies and preferences/intolernaces. Plan of Correction: The following corrective action was provided by the director during the exit interview: The director immediately corrected the allergy documents for the entire center.
