Comments:
An unannounced monitoring inspection was conducted this date. A total of 88 children and 14 staff on site. All 10 classrooms were observed. Children were busy with cutting out shapes and gluing them on construction paper. Walls were decorated with hand painted leaves completed by children. In the Beginners class the children were sitting in circle time with various instruments, along with staff and were preparing to listen to music and while using their instruments. In the intermediate class, the children were also in circle time going over the day of the week and weather. In the infant room, footprint ghosts were on display. A total of 5 children's records and 5 staff records were reviewed and found complete. Outside transition to inside was observed of the Intermediate B classroom. Thank you for your cooperation today. Standard #: 22VAC40-185-50-A
Description: Based on review of the center's classrooms, information pertaining to children has not been kept confidential. Evidence: Allergy lists for children are posted in classrooms including the child's name and what allergy they have. This information is available to the general public.

Plan of Correction: We have an allergy cover sheet that will be placed over the list of allergies in each classroom.

Standard #: 22VAC40-185-280-B
Description: Based on observation of the Toddler A class, a hazardous substance was found not locked. Evidence: An insulin syringe and insulin was found in an unlocked refrigerator in the Toddler A classroom.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-185-500-A
Description: Based on observation of the Infant A class, hand washing procedures were not followed. Evidence: The one staff in Infant A was observed changing an infant's diaper and failed to to wash hands directly afterwards and proceeded to pick an infant who was crying.

Plan of Correction: Staff will be retrained on the diapering procedures today.

Standard #: 22VAC40-185-520-C
Description: Based on observation of the Intermediate A class bathroom, diaper ointment was found accessible to children. Evidence: The bottom drawer of the storage cart contained topical ointments which were accessible to children.

Plan of Correction: Will place the ointments in the cabinet that is not within reach.
