Comments:
An inspection was conducted in response to a call received from the center to report that on August 16, 2018, a toddler had been left alone in a classroom for a short period of time. Video footage of the classroom and playground were observed, interviews with staff were conducted, and staff files were reviewed. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on interviews, the center did not ensure that their activities and services were maintained in compliance with their own policies and procedures for identifying where attending children are at all times. Evidence: 1. The center has established procedures that require staff, in each class, to use a tablet to check off names of the children whenever they pass through a threshold. 2. Staff #3 stated that on August 16, 2018, she had the tablet and she thought she checked the children off when leaving the classroom to go to the playground. She stated she did not perform name to face check off once they were outside on the playground.

Plan of Correction: Re-training of procedures for Face to Name. Unannounced FTN observations done in all age groups weekly.

Standard #: 22VAC40-185-240-A
Description: Based on record review and interviews, not all staff received training by the end of their first day of assuming job responsibilities. Evidence: 1. Staff # 1 stated she did not have training and staff #1's file did not include documentation of training on staff member's responsibilities, related policies and procedures, the center's playground safety procedures, recognizing and reporting child abuse and neglect, confidential treatment of personal information about children in care and their families, and the standards that relate to the staff member's responsibilities. 2. Staff #2 stated she was put into the classroom the day she filled out new hire paperwork, and that she did not receive training about job responsibilities. Staff #2's file gives their start date as August 18, 2017. This file did not contain documentation of training on staff member's responsibilities, related policies and procedures, and the center's playground safety procedures, recognizing and reporting child abuse and neglect, confidential treatment of personal information about children in care and their families, and the standards that relate to the staff member's responsibilities.

Plan of Correction: At October Staff Meeting: go over New Hire packet, answer any questions, have staff sign and date.

Standard #: 22VAC40-185-340-A
Description: Based on review of the center's video recordings and interviews, when supervising the children, the staff did not always ensure their care, protection, and guidance. Evidence: 1. On August 16, 2018, staff left child #1 (21 months) alone in the classroom for approximately 2 and 1/2 minutes between 11:23 and 11:26 AM when staff and the toddler group exited their classroom to go to the playground. 2. At the time of the incident, Staff #1, #2, and #3 were supervising 10 children in the toddler room ( age 16 months-24 years). The 3 staff confirmed they did not realize that child #1 did not go outside with the rest of the class. 3. A parent notified Staff #2 that child #1 was alone in the classroom. Staff #2 then went inside to bring child #1 outside.

Plan of Correction: Unannounced FTN observations done in all age groups weekly. Ongoing

Standard #: 22VAC40-185-340-F
Description: Based on interviews and the center's video footage, the center failed to ensure that children under 10 years of age shall always be within actual sight and sound supervision of staff. Evidence: 1. On August 16, 2018, at approximately 11:23 AM, child #1 ( 21 months of age) was left alone and without actual sight and sound supervision of staff for approximately 2 1/2 minutes. The classroom and the playground video footage, and staff interviews confirmed child #1 was left alone in the classroom when all 3 staff went outside with the toddler group. 2. The video recorded children and staff exiting the toddler classroom, and after the group leaves the classroom, child #1's head can be seen at the bottom of the screen. 3. At the time of the incident, Staff #1, #2, and #3 were supervising 10 children in the toddler room ( age 16 months-24 years). The 3 staff confirmed they did not realize that child #1 did not go outside with the rest of the class. 4. Staff #1 stated she had gone out with 5 children before the rest of the class went outside and was responsible for supervising those toddlers during water play. Staff #2 and staff #3 stated that they went outside, from the classroom, with the remaining children. Staff #3 had the Head to Face count sheet and was the last staff member to exit the room behind the children. 5. A parent notified Staff #2 that child #1 was alone in the classroom. Staff #2 then went inside to bring child #1 outside.

Plan of Correction: Unannounced FTN observations in all age groups/classroom weekly. Re-train how to check the classroom or playground prior to leaving location.
