Comments:
A Subsidy Health and Safety Inspection was conducted on Friday, October 5, 2018 from 10:00am to 12:10pm to review supplemental health and safety requirements. The center?s director assisted the inspector throughout this inspection. The census for today?s inspection consisted of 47 children in care and 15 staff. During the inspection, the licensing inspector observed the children playing in centers and interacting with peers and the staff. The children were also observed taking breaks and washing their hands. The staff were observed having positive interactions with the children. The following information was reviewed: daily attendance, required postings, injury reports, and liability insurance policy. The first aid kit and non-medical emergency supplies were inspected and found to be in compliance. The most recent evacuation drill was documented on 10/03/2018. The last shelter-in-place drill was documented on 09/17/2018 and previously on 08/14/2018. The last documented lockdown drill was on 06/27/2018. The last documented health inspection occurred on 03/31/2018. The last documented fire inspection occurred on 09/14/2018. Five staff records and five children?s records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Standard #: VENDSUB-000-012-E
Description: Based on a review of five staff records and interview on 10/05/2018, the center did not ensure completion of First Aid and CPR training of four out of five staff within 30 days of the date of employment. EVIDENCE: 1) In Staff #1's record (DOH: 08/10/2018), there was no certificate of completion for First Aid/ CPR found. 2) In Staff #2's record (DOH: 08/27/2018), there was no certificate of completion for First Aid/CPR found. 3) In Staff #3's record (DOH: 08/20/2018), there was no certificate of completion for First Aid/CPR found. 4) In Staff #4's record (DOH: 08/15/2018), there was no certificate of completion for First Aid/CPR found. 4) Administrative staff confirmed Staff #1, Staff #2, Staff #3, and Staff #4 have not completed First Aid/CPR training.

Plan of Correction: Per the Director: Will schedule. FA/CPR training will be scheduled within 2 weeks for needed staff.
