Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on October 15, 2018 from 10:35 a.m. through 1:30 p.m. There were twenty-eight children in the care of six staff. Five children's and eight staff records were reviewed and updated. No medication is being administered at this time. Areas of non-compliance may be found on the violation notice. Areas observed were: educational center activities, free choice, outside play, hand washing/bathroom and diapering procedures, transition, and nap. Interactions between the children and staff were observed as appropriate and the children seem comfortable with the daily routine and schedule. Standard #: 22VAC40-185-130-A Description: Based on record review, the center failed to obtain proof of immunization before one child entered the program. Evidence: 1. Five children's records were reviewed. 2. Child #4 did not have documentation of immunizations before the first date of attendance. 3. First date of attendance was documented on 10/10/2018. 4. The assistance director confirmed there was not documentation of immunizations. Plan of Correction: The center will contact the parent to provide immunization documentation.
