A Subsidy Health and Safety inspection was conducted on 10/15/18 to review supplemental health and safety requirements. Today's visit was a routine monitoring inspection. the inspection started at 9:30pm and concluded at 4:00pm. There were one hundred one children in care between infant and school age with fifteen staff members. During the inspection all nine classrooms were observed during the inspection, infant care was observed, the outdoor playgrounds were viewed, three out of ten medications were viewed along with authorization, an injury report was viewed, twelve out of twenty-seven staff records were reviewed, seven out of one hundred fifty-seven children's records were reviewed, emergency supplies were viewed, and some policies and procedures were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-009-2 Description: Based on the inspector's review of an injury report, the center failed to ensure a report regarding a serious injury regarding outside medical treatment was made with the department. Evidence: The inspector reviewed all documentation regarding and injury of a child dated 8/7/18 while the child was under the center's supervision while on a field trip. The child sustained an broken bone and it was required that the child receive medical treatment as a result of the injury. The department was not notified of the injury as required by this requirement. Plan of Correction: The Program Director will ensure all future injuries requiring outside medical treatment are reported Via the DSS online reporting form.
