Discussed with the Director: Expired Epipens due to shortage - have parents document. Also discussed getting the new Sworn Statement form from your area manager and make sure that all staff complete that. Also check on CPS and Florida CPS check for 1 new staff.

An unannounced monitoring inspection was conducted beginning at 3;50 and ending at 5:50. There were 45 children present with 4 staff. Children were observed upon arrival, going outside on the playground, having snack and at homework time. Reviewed 10 student records - see violation notice Reviewed 6 staff files Reviewed 10 accident reports - see violation notice. Description: Based on observation, the License that was posted on the parent board is the previous expired license. Evidence: The License that is posted is an expired one. The current license is not posted

Plan of Correction: The Director will notify the Area Manager.

Standard #: 22VAC40-185-60-A
Description: Based on a review of 10 children's records, it was determined that not all records were complete. Evidence: Child D has a Parent Agreement that is not signed; Child G who enrolled on 8/27/18 did not have a physical or shot record; child H has a physical and shot record that are not signed by the physician or designee.

Plan of Correction: The Site Director will go through all records to assure complete files.

Description: Based on a review of 10 Accident Reports, it was determined that not all forms were completed. Evidence: 7 of 10 forms did not have the time that parent was notified of the accident and 1 of 10 did not have time of the accident.

Plan of Correction: The Site Director will retrain all staff in completing forms correctly.
