Comments:
An unannounced Renewal Inspection was conducted on October 3, 2018. At the time of entrance there were 26 children and 10 staff members present. Additional staff were on site for administrative purposes and program support. The sample size consisted of 5 children's records and 5 staff records. The inspection began in the center began approximately 9:00 am and ended at 11:45 am. The background checks for the agents/ applicant for the center were reviewed at the corporate office from approximately 1:00 pm unit 2:00 pm. Children in the center were observed listening to music, dancing and playing outside. The inspection results were reviewed with the center director during the exit interview and the agents/applicant regarding the background checks. Standard #: 22VAC40-185-240-C
Description: Based on record review and staff interviews, the licensee did not ensure program directors and staff who work directly with children shall annually attend 16 hours of staff development activities that shall be related to child safety and development and the function of the center. Evidence: On October 3, 2018, the following was found during record review: 1. The record for staff #2 only included documents for review of 15 hours of annual training. 2. The record for staff #4 only included documents for review of 12 hours of annual training. 3. The record for staff #5 only included documents for review of 8 hours of annual training.

Plan of Correction: All staff are up to date on training starting 1/2018 to now. Director will be monitored training often.

Standard #: 63.2-1721.1-B-3
Description: Based on record review and staff interviews, the licensee did not ensure 1 of 5 records have a copy of the results of a search of the central registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him. Evidence: On October 3, 2018, the record for board member #4 did not include a search of the central registry for the state outside of VA where the board member lived within the last 5 years.
