An unannounced Subsidy Health and Safety Inspection was conducted on October 15, 2018 from approximately 2:10 pm until 4:15 pm. At the time of entrance there were 13 children and 4 staff members were present with children. There were additional staff on site for administrative purposes. The sample size consisted of 5 children's records and 3 staff records. Children were observed eating snack and during story time. Areas of non-compliance are identified on the violation notice. The results of the inspection were reviewed with the Center Cooridinator. Please the licensing inspector, Christine Mahan (757) 404-0568 with any questions.
Standard #: VENDSUB-000-004-F Description: Based on observation and interviews, the vendor did not ensure to post, with parental approval, or keep in a way that is immediately accessible to staff, a current list of all children?s allergies, sensitivities, and dietary restrictions. Evidence: On October 15, 2018, there was a list in classroom #12 and #13 of 3 children's names listed with dietary restriction and the center had not obtained the parental permission or kept it in a manner that it was not visible to the public. Plan of Correction: The center director fixed this during inspection.
