63.2 Licensure and Registration Procedures

Comments:
An unannounced inspection was conducted in the center today. At the time of entrance there were 2 staff present with 6 preschool age children. The sample size consisted of 5 children's records and 2 staff record. Children were observed during reading time, eating snack and playing centers to include coloring, puzzles and dramatic play. No medications are being administered. The playground was not inspected due to inclement weather. The inspection began at approximately 9:55 am and ended at 11:45 am. Standard #: 22VAC40-185-60-A
Description: Based on record review and staff interviews, in two of five children's records reviewed, the licensee did not ensure that each child's record was maintained with all required information. Evidence: During the inspection conducted on October 16, 2018, the following information was not documented in each child's record: 1. The record for Child #1 did not include the home phone number for both parents listed, the employment phone number for one parent listed, any additional program attending and the address for the second required emergency contact. 2. The record for Child #2 did not include the home address, home phone number , employment phone number for one parent listed, the home phone number for one parent listed, previous childcare, additional programs attending, the phone number for one emergency contact and did not have a phone number and home address for the second required emergency contact.

Plan of Correction: The center director will have the records updated.

Standard #: 22VAC40-185-260-B
Description: Based on documentation review and staff interviews, the licensee did not obtain annual approval from the health department. Evidence: During the inspection conducted on October 16, 2018, the most recent health inspection available for review was dated 9-12-17.

Plan of Correction: The center director will get a copy of the inspection soon or have it conducted.

Standard #: 22VAC40-185-530-A
Description: Based on documentation review and staff interviews, the licensee did not ensure there shall be at least one staff member trained in first aid, cardiopulmonary resuscitation, and rescue breathing as appropriate to the age of the children in care who is on the premises during the center's hours of operation and also one person on field trips and wherever children are in care. Evidence: During the inspection conducted on October 16, 2018, there was not documentation of at least one staff member having current CPR and First Aid. The record from Staff #1 has CPR and First Aid that expired in September 2018.

Plan of Correction: The center director will get copies of the certifications for CPR and First Aid.

Standard #: 22VAC40-185-550-D
Description: Based on documentation review and staff interviews, the licensee did not ensure to implement a monthly practice evacuation drill and a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios. Evidence: During the inspection conducted on October 16, 2018, there was not documentation of evacuation drills conducted in May 2018 or June 2018.

Plan of Correction: The center director will make sure to document and conduct drills in the future.

Standard #: 63.2-1720.1-B-2
Description: Based on record review and staff interviews, the licensee did not ensure two of two staff records has a Office of Background Investigations (OBI) letter stating eligibility for hire in response to the national fingerprint check prior to beginning employment. Evidence: During the inspection on October 16, 2018, the records for Staff #1 and Staff #2 did not include the OBI letter of eligibility for hire . Staff #1 and Staff #2 were observed working during the inspection.

Plan of Correction: The center director will get documents of the OBI checks and put in the staff records.
