Comments:
An unannounced monitoring inspection was initiated today. The physical plant, 5 staff files, 5 children's files, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected between 11:30-12:45pm. There were 30 children under the supervision of 7 staff. During the inspection, children were observed playing outside on the playground, reading books, listening to storytime, singing songs, cleaning up for and eating lunch. For lunch, the children were served rainbow tortellini pasta with kale pesto, baby peas, cantaloupe and milk. The interactions between the children and staff were seen to be positive and constructive in nature. Should you have any questions, please contact me at mary.wizbicki@dss.virginia.gov.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, there was an area of the center which was not maintained in a safe condition. Evidence 1. The fence that surrounds the playground has a wooden slat which has broken off. This created a sharp edge which could entangle clothing or skin or be an entrapment hazard.

Plan of Correction: We will have someone come out to fix this.

Standard #: 22VAC40-185-320-B
Description: Based on observations, there was a restroom which was not equipped with all required items. Evidence 1. In the downstairs boy's bathroom, there was no soap.

Plan of Correction: Corrected during the inspection.
