Comments:
An unannounced monitoring inspection was conducted on Wednesday, October 3, 2018 from 4:20pm to 5:37pm to determine the center?s compliance with licensing standards. The center?s site director assisted the inspector throughout the inspection. The census for today?s inspection consisted of 117 children in care. During the inspection, the licensing inspector observed the children in the cafeteria eating dinner, interacting with peer and staff. One group of children was formally observed in the gym playing basketball and during free-play. Another group of children was observed completing an art activity. The children were also observed taking bathroom breaks. The staff were observed having positive interactions with the children. The first aid kit and non-medical emergency supplies were inspected and found to be in compliance. During the inspection, the required postings were reviewed and determined to be in compliance. There were ten children?s records reviewed. Additional components of staff records are stored off site and will be reviewed by licensing inspectors at a later date. Standard #: 22VAC40-185-140-A
Description: Based on a review of 10 children's records on 10/03/2018, the center did not ensure one child obtained documentation of a physical examination by or under the direction of a physician prior to attendance or within one month after attendance. EVIDENCE: 1) The record for Child #3 (DOA: 08/21/2017) did not contain documentation of a physical examination.

Plan of Correction: The provider?s responses for the ?plan of correction? were not received as of 10/17/2018 and will not appear on this Violation Notice.

Standard #: 22VAC40-185-60-A
Description: Based on a review of 10 children's records on 10/03/2018, the center did not ensure that two of 10 children's records reviewed contained two complete emergency contact records. EVIDENCE: 1) The record for Child #1 (DOA: 08/31/2018) did not contain the phone number for the two emergency contacts listed. 2) The record for Child #7 (DOA: 11/01/2017) did not contain the address for the two emergency contacts listed. Each child record shall contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Description: Based on staff interview on 10/03/2018, the center did not ensure that the required information for each staff was kept at the center. EVIDENCE: 1) During the inspection, Staff #1 stated that the name, address and telephone number of a person to be notified in an emergency for each staff was not at the center.

Standard #: 22VAC40-185-550-E
Description: Based on staff interview on 10/03/2018, the center did not ensure that a record of the dates of monthly evacuation drills and two shelter-in-place drills per year were maintained for one year.

EVIDENCE: 1) During the inspection, Staff #1 stated the drills were completed every month, but the practice log was not available for the licensing inspector to review on 10/03/2018.
