An unannounced inspection was completed on 10/17/2018 from 1:20 P.M.-6:00 P.M. At the time of the inspection 54 children were in care with an additional 45 children arriving afterschool. Thirteen staff and two administrators were present. Nine children records, nine staff records, four medications and documentation, four injury reports, first aid kits, emergency supplies, playground, and posted required information were reviewed. Children were observed during naptime, diapering, handwashing, free choice activities, snack time, after school arrival, homework time, and outdoor play. Standard #: 22VAC40-185-280-B Description: Based on observation, the center failed to ensure that all hazardous materials were kept in a locked location. Evidence: 1. In the bathroom of the Four Year Old Classroom the cabinet above the toilet was unlocked and contained Lysol All Purpose Cleaner, Dollar General Disinfect Spray, and Glass Multi Surface Cleaner. 2. The warning labels stated "keep out of reach of children", "hazardous to human and animals", and "flammable". 3. Staff verified that the cabinet was unlocked. Plan of Correction: The administrator will review the standards regarding hazardous materials with all staff.
Standard #: 22VAC40-185-290-3 Description: Based on observation, the center failed to ensure that all outlets had protective covers. Evidence: 1. In the Three Year Old Classroom there was a power strip next to the microwave that had two outlets that were not covered with protective covers. 2. In the Toddler Classroom there was an electrical outlet next to the play kitchen that did not have a protective cover. 3. Staff verified that the outlets did not have protective covers. Plan of Correction: The teachers immediately placed protective covers in the outlets.
Standard #: 22VAC40-185-510-E Description: Based on review of non-prescription medications , the center failed to ensure that all medications were labeled with the dosage amount and time or times to be given. Evidence: 1. Child 1M had a bottle of Infant Tylenol Pain Reliever that was not labeled with the dosage amount and the time or times to be given. 2. Child 2M had a bottle of Infant Tylenol Pain Reliever that was not labeled with the dosage amount and the time or times to be given. 3. Staff verified that the Tylenol Pain Reliever was not labeled with the dosage amount and the times or times to be given. Plan of Correction: The staff will label the medication and in the future will make sure that all medication is labeled with the child's name, dosage amount, and the time or times to be given.
Standard #: 22VAC40-185-520-A Description: Based on a review of diaper ointments and documentation, the center failed to ensure all over the counter skin products will not be kept or used beyond the expiration date of the product. Evidence: 1. In the Toddler Classroom there was a container of Desitin that had an expiration date of 06/2018. The diaper ointment had been applied on 09/19/2018 and on 10/02/2018. 2. Staff verified that the diaper ointment had expired, and had been applied to the child. Plan of Correction: The staff will return the Desitin to the parent the next day the child is in care, and review all diaper ointment expiration dates on a regular basis.
Standard #: 22VAC40-185-520-C Description: Based on a review of diaper ointments, the center failed to label all diaper ointments with the child's name. Evidence: 1. In the Toddler Classroom there was a bottle of Boundreauz Buttpaste, Aquophor Healing Ointment, A&D Diaper Ointment, and Desitin that were not labeled with the child's name. 2. Staff verified that the diaper ointments were not labeled with the child's name. Plan of Correction: The administrator will review the standards regarding diaper ointments.
