Comments:
A Subsidy Health and Safety inspection was conducted from 3:09 PM to 6:15 PM for the purpose of reviewing supplemental health and safety requirements. Today, there were twenty-one elementary school age children, supervised by two to three staff after school. Five volunteers assisted with the children. I viewed compliance with operational responsibilities, record keeping, children's information, staff training, hazardous substances, behavior guidance, hand washing, emergency drills, emergency planning, food allergy information, nutrition and food service, two staff records, training for a third staff member, and seven records for children. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. A violation was found during the supplemental subsidy health and safety inspection and is identified on the violation notice.

New subsidy requirements became effective on October 17, 2018. The facility has not yet complied with the following requirements: All staff who work with children must be informed of children's allergies. Also, procedures for the continuity of operations to ensure that essential functions are maintained during an emergency must be included in the emergency response plan. The facility is to review the new requirements and work with their assigned inspector to ensure future compliance. Let me know if you have any questions. 540/430-9259

Standard #: VENDSUB-000-004-F
Description: Based on a review of children's allergies, seven children's records regarding allergies, the location indicated for a list of children and food allergies, and interviews with staff, the center did not have a current list of all children?s allergies, sensitivities, and dietary restrictions kept in a way that is immediately accessible to staff. (Repeat violation.) Evidence: During interviews with staff, staff member 1, who served snack, indicated that she had kept a list of names and types of food allergies for children. However, the list was not found in the location which is only accessible to staff. Staff member 2 and 3 were not aware that any children had food allergies. During a record review of two children with staff member 1, child 7 had a diagnosed food allergy. There was not any documentation of a food allergy for child 6 and staff member 1 indicated that a parent provided the information verbally.

Plan of Correction: Administrative staff will make sure a current list of any child's food allergy, sensitivity and dietary restriction is kept in a designated location that is always accessible to staff. Administrative staff will make sure that all staff are aware of children's food allergies and any food restrictions.
