An unannounced mandated inspection was completed on 10/22/2018 from 9:30 A.M. until 2:00 P.M. At the time of the inspection 43 children were in care with eight staff and one administrator present. Six children's records, seven staff records, required posted information, evacuation drills, emergency plan, one medication, six injury reports, first aid kits, emergency supplies, and outside play area were reviewed. Children were observed during transition times, handwashing, snack time, story time, outside play, small group activities that included a seasonal bingo game, rhyming activity, literacy activity, behavioral guidance, lunchtime, diapering, and naptime. Standard #: 22VAC40-185-550-M Description: Based on review of injury reports, the center failed to ensure that the injury reports had all required information. Evidence: 1. Two out of six injury reports did not document a plan of preventing recurrence. 2. Staff verified that the injury reports did not contain a plan of preventing recurrence. Plan of Correction: The administration will review with all staff the proper way to complete injury reports.
