A written request to modify the license has been submitted, the program is requesting to increase their capacity from 72 to 90. The center's response for the plan of correction was not received as of 10/22/2018, and will not appear on this Violation Notice.?

Today an unannounced monitoring inspection was conducted between the hours of 3:10pm and 5:00pm. I observed 69 children directly supervised by 5 staff. Ratios and supervision were in compliance. Children were observed watching a movie, doing homework, doing crafts, coloring, and participating in group play. Staff were actively engaged with the children and assisting them with tasks as needed. The physical plant, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. The cafeteria was found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. Medication management was in compliance. Required staff training documents were on-site. Emergency preparedness supplies in order. Required documents posted. Record keeping was in compliance. Standard #: 22VAC40-185-550-M Description: Based on record review, documentation of two children's injury/incident reports were found to be incomplete. Evidence: Two injury/incident reports completed were not filled out properly, an injury report dated 8/23/2018 was missing the date and time of parent's notification, an injury report dated 8/30/2018 was missing information regarding future actions to prevent recurrence.
