63.2 Licensure and Registration Procedures
An unannounced monitoring inspection was completed today 10/24/2018. There were 23 children in care on the day of the inspection with seven staff. Five children's and Seven staff's records reviewed during the inspection. A physical plant walk through was completed, and observations of the children. The inspector arrived at 8:20 am and departed the program at 12:30 pm. Discussion with licensee about emergency plan size group and transportation plan.
Standard #: 22VAC40-185-210-A Description: Based on review of staff's record, the center failed to ensure that all program leader had all requirements. Evidence: 1. Staff # 5 was working as program leader with the two year olds and the record did not indicate that she was a program leader. She had the required 24 hours needed for program lead, with the 6 month experience. Her job title indicated that she was a Teacher Assistant and she did not have a her high school diploma or equivalent. Plan of Correction: The owners will change the job title and will have the staff person to bring in the high school diploma.
Standard #: 22VAC40-185-320-F Description: Based on observation of the two year old room, the center failed to ensure that a platfrom or a set or steps shall be provided for children using the toilet. Evidence: 1. Observed in the in the two year old classroom, two children using the adult sized toilet. They had to be placed on the toilet with help by the teacher. There was not platform or step to allow the child to gain access themshelves. Plan of Correction: Staff will be reminded to allow the children to use the step stool that was under the sink in the restroom.
Standard #: 63.2-1720.1-B-3 Description: Based on review of seven new staff's records, the center failed to ensure that all staff that lived out of state within the past five years obtained a out of state CPS check. Evidence: 1. Staff # 4 lived in Colorado within the past five years and the center did not complete an out of state CPS check in that state as required by the Code of VA. Plan of Correction: The center owners will follow the procedures on the website to obtain the out of state CPS check ASAP. The owner will in the future will ensure all new staff will have all new have the correct forms.
