22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted on October 24, 2018, between the hours of approximately 9:40 a.m. and 2:15 p.m. Twenty-nine (29) children were enrolled and twenty-seven (27) children were in the care of four staff members, in two classrooms, during the on site inspection. Five children's records were reviewed and five staff records were reviewed and/or updated. Two children's medications and accompanying authorization forms were reviewed/observed. Posted information (license, licensing inspection documentation, emergency evacuation diagram and plan notification, emergency telephone numbers, menus, asbestos notification and classroom schedules), safety equipment (first aid kit, flashlight and radio) and available documents (emergency evacuation drill/shelter-in-place drill record, attendance records, insurance, local agency reports and asbestos surveillance) were reviewed. The daily schedules and menus accurately reflected the activities and snacks provided. Snacks, on this date, consisted of cheese, crackers and milk (4-year-old classroom) and Oreo cookies, pretzels and milk (3-year-old classroom). Program observation was completed and included: free choice time - coloring, puzzles, playdoh, block play, toys, and concept development/counting; group time; transitions; snack; storytime; handwashing; outdoor play and dismissal/pick-up of children. Staff/child interactions were observed to be positive and supportive of children's growth and development. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Procedures were observed to be in place to ensure the safety of the children in care (head counts and dismissal/pick-up procedures). Inspection findings were discussed and exceptions to compliance of licensing standards were cited on the accompanying Violation Notice. Thank you for your time and assistance. Standard #: 22VAC40-185-70-A Description: Based on review of five staff records and discussion with staff on October 24, 2018, the program failed to obtain written information to demonstrate that one staff member possessed the education required by the job position (program director). Evidence: 1. Review of staff #1's record, on this date, determined that documentation of the staff member's education was not on file (college transcript, teaching certificate, etc.). 2. Staff #1 and #5 confirmed the lack of documentation in the staff record. Plan of Correction: Based on discussion, verification of education will be provided and included in the staff record.
Standard #: 22VAC40-185-240-D-5 Description: Based on review of five staff records and discussion with staff on October 24, 2018, the program failed to ensure at least one staff member on duty was trained in daily health observation of children. Evidence: 1. Review/update of four staff records, on this date, determined that the certification of the only staff member trained in daily health observation expired on October 21, 2018. 2. Staff #5 confirmed that staff members were not currently trained in daily health observation of children. Plan of Correction: Staff will be trained in daily health observation of children.
Standard #: 22VAC40-185-510-E Description: Based on observation and discussion with staff on October 24, 2018, the program failed to ensure one child's medication was correctly labeled. Evidence: 1. Observation of child G's medications, on this date, determined that one medication was not labeled with the medication dosage and the time to be administered. 2. Staff #1 and #4 confirmed the lack of labeling on the child's medication. Plan of Correction: The medication was correctly labeled during the inspection. In the future, medications will be labeled with the dosage and time(s) to be administered.
