Comments:
A joint inspection was conducted on September 6, 2018 with Loudoun County Sheriff and Loudoun County Child Protective Services in response to a self reported incident from the management team at the Primrose School of South Riding. The inspection included interviews with the child, management team, a review of written staff statements, and a review of staff and children's records. . Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-410-1
Description: Based on interviews and written statements, one staff member engaged in a forbidden action resulting in injury to a child. Evidence: 1. On 9/4/2018 at approximately 10:30am Staff 1 was observed correcting a child's behavior by pulling a child seated on the ground up by one hand. The child sustained a dislocated elbow as a result of the teacher's action.

Plan of Correction: While we believe this was an isolated incident without any malicious intent, Staff member 1 was immediately put on administrate leave and later dismissed. All staff members have been retrained on the forbidden actions standards as well as safe handling of children. The Leadership Team will continue to carefully monitor staff/child interactions.
