Comments:
Today an unannounced Monitoring Inspection took place between the hours of 11:40am and 2:25pm. At the start of the inspection there were a total of 6 children with 2 staff; all ratios were incompliance. A total of 5 children?s files and 3 staff files were reviewed. The children were observed playing freely, eating lunch (provided from home) and napping. If you have any questions or concerns, please feel free to contact me at whitney.mcgrath@dss.virginia.gov. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, the center did not obtain documentation of immunizations for each child before the child can attend the center. Evidence: Child #4 file does not contain documentation of immunizations.

Plan of Correction: I'll get this within 2 weeks.

Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not obtain documentation of a physical exam within the first 30 days of attendance for each child. Evidence: Child #3 (first date of attendance: 7/16/2018), Child #4 (first date of attendance: 9/17/2018) and Child #5 (first date of attendance: 9/4/2018) files do not contain documentation of physical exams.

Plan of Correction: I'll get this within 2 weeks.

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records did not contain all required information. Evidence: Child #2 file did not contain addresses for both emergency contacts. Child #3 file did not contain documentation that the child's age and identity had been verified. Child #4 file does not contain any contact phone numbers for one parents, two emergency contacts or documentation that the child's age and identity had been verified. Child #5 file does not contain addresses for both emergency contacts or documentation that the child's age and identity had been verified.

Plan of Correction: I will have the parents come in and provide the missing information.

Standard #: 22VAC40-185-70-A
Description: Based on record review, staff records do not contain all required information. Evidence: Staff #1 file does not contain documentation of a second reference check. Staff #2 and Staff #3 files do not contain addresses for their emergency contacts or documentation of two reference checks.

Plan of Correction: I'll have them ready by the end of next week.

Standard #: 22VAC40-185-440-E
Description: Based on observation, occupied cribs are not at least 12 inches apart. Evidence: In the back corner of the infant sleeping area there are two cribs approximately 2 inches apart. Both cribs were observed with children sleeping in them.

Plan of Correction: We can turn the crib.

Standard #: 22VAC40-185-500-A
Description: Based on observation, staff did not wash children's hands with soap and running water or disposable wipes before and after eating meals. Evidence: At the start of the inspection the children were observed playing freely in the classroom. At 12:00pm 3 children were placed in high chairs and two sat down at a table to eat lunch. The children's hands were not washed prior to eating or after they were finished eating.

Plan of Correction: We will put a reminder note on the wall for the caregivers.

Standard #: 22VAC40-185-520-C
Description: Based on observation, diaper ointment was not labeled with each child's name. Evidence: There are three diaper ointments stored on the changing table that do not contain any names to identify their owners.

Plan of Correction: This was corrected during the inspection.

Standard #: 22VAC40-185-570-B
Description: Based on observation, bottles are used in a child's sleep location. Evidence: Child #1 was observed falling asleep in their crib while holding and drinking from a bottle.

Plan of Correction: This was corrected during the inspection.

Standard #: 22VAC40-185-570-E
Description: Based on observation, bottles were not dated and/or labeled with each child's name. Evidence: Child #1 had two bottles in the refrigerator without today's date, Child #2 had two bottles in the refrigerator without today's date, Child #3 had one bottle in the refrigerator without today's date. There are four bottles of frozen milk in the freezer of the refrigerator that do not contain a name or date.

Plan of Correction: I'll start keeping tape and a marker by the fridge so that parents can label daily.

Standard #: 22VAC40-191-60-C-2
Description: Based on observation, the center has not obtained central registry findings within 30 days of employment for two employees. Evidence: Staff #2 and Staff #3 files do not contain documentation of central registry results. The program owner documented that the background checks were mailed out on 9/14/2018, however results have not been received. The program owner has stated that she has not yet called to follow up regarding the whereabouts of the results.

Plan of Correction: I'll call later on today to find out the status of these checks.

Standard #: 63.2-1720.1-B-2
Description: Based on record review and interview, the center has not obtained fingerprint results for one employee. Evidence: Staff #3 file does not contain documentation of an eligibility letter. Staff #3 was observed working with the children today.

Plan of Correction: I can make the appointment for her today to get this done.
