Comments:
A Subsidy Health and Safety inspection was conducted on 10/25/18 from 10:15am to 1:00pm to review supplemental health and safety requirements. At the time of entrance there were 47 children ages four months to four years old in care with ten staff. Children were observed participating in various activities in the classroom, playing outside, eating lunch and during rest period. Records were reviewed for five children and five staff during the inspection. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. New subsidy requirements became effective on October 17, 2018. The facility has not yet complied with the following requirement(s): 22VAC40-665-540 TB screening. The Licensing Inspector reviewed this requirement with the facility to ensure future compliance.

Standard #: VENDSUB-000-012-E-1
Description: Based a review of five staff records, it was determined that the vendor did not ensure that all staff have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. Evidence: 1. The record for staff #1 did not contain documentation for a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. 2. The record for staff #5 did not contain documentation for a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. 3. Staff #6 (Program Director) reviewed the record for staff #1, and confirmed that there was not documentation to demonstrate that staff #1 had a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care.

Plan of Correction: The vendor responded: Staff #1 will complete a course to obtain certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. Current staff will complete a renewal first aid certification course prior to the expiration of their current certification. All new staff will complete a course to obtain certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care within 90 days of employment.

Plan of Correction: The vendor responded: Staff #1 will complete a course to obtain certification in first aid appropriate to the age of children in care. Current staff will complete a renewal first aid certification course prior to the expiration of their current certification. All new staff will complete a course to obtain certification in first aid appropriate to the age of children in care within 90 days of employment.
