22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today, October 23, 2018 as a follow-up to the IPOC (Intense plan of correction) that was submitted to the department on September 6, 2018. Some but not all standards were reviewed during today's inspection. Upon the inspectors arrival, the infant and toddler class were combined for nap time (see violation notice). The preschool classroom and two's classrooms were observed napping. The school age children were not in attendance during today's inspection. All required information was posted to include the license, last inspection notice, emergency numbers, menu, evacuation maps/procedures and daily schedules. Three staff records were reviewed. Today's Ratios Observed: Cat 2: (2-3 years) 10 children with 1 staff. (nap time) Butterflies: (3-5 years) 16 children with 1 staff. Cuddle Bugs/ Cat 1: (Infants-toddlers) 6 children with 1 staff. Standard #: 22VAC40-185-350-E-1 Description: Based on observation, the facility did not maintain the staff to child ratio required for children birth to the age of 16 months: one staff member for every four children. Evidence: Six infants were observed with one staff in the infant classroom. Plan of Correction: Corrected while the inspector was on site. An additional staff returned from break. The assistant director stated, Staff will review procedures for breaks and will not be allowed to leave a classroom until they have spoken with the administrator and coverage is available.
