Comments:
The licensing inspector conducted an unannounced renewal inspection, at which play groups, outdoor time, personal hygiene, activities, and lunch time were observed. Children were observed; and staff and children files and other documentation were reviewed. Twenty-seven (27) children, aged 2 months-5 years, were found to be in care with 5 staff, and office staff. Previous violations were reviewed, and one (1) of the violations from the previous inspection had not been corrected. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 8:20am and concluded at 12:30pm.

Violations:
Standard #: 22VAC40-185-40-E
Description: Based on review of the classrooms and other areas the facility failed to maintain compliance with this standard as required relating to terms of the license. Evidence: The classrooms and other areas both inside and outside looked unkempt/in disarray. There were not many visible toys for the children in the classrooms, the rooms looked somewhat bare. The rooms also didn't have all of the documentation posted as required.

Plan of Correction: Facility had painting done over the past weekend, so the required postings had not yet been re-placed on the walls. Staff will be reminded of the requirements to up keep their rooms in a clean and organized manner, and to visually promote play.

Standard #: 22VAC40-185-330-B
Description: Based on review of the playgrounds the facility failed to maintain compliance with this standard as required relating to the resilient surfacing. Non-compliance with this standard was cited on the last inspection dated 5/25/2018, and therefore is marked as repetitive. Evidence: The depth of resilient surfacing under the school aged climber did not meet the ASTM depth guidelines as required. There was approximately 3 inches under and around the climber.

Plan of Correction: The mulch was replaced shortly after the May inspection, but with the damage and flooding of the last 2 weather issues the mulch has been displaced from under and around the bigger climber. We are working towards getting new mulch to meet compliance.

Standard #: 22VAC40-185-340-F
Description: Based on observations during the inspection the facility failed to maintain compliance with this standard as required relating to supervision of the children. Evidence: The inspector witnessed a 3 year old child exit a classroom and walk down the hall towards another classroom door. This child was unattended by staff or any other adult. The inspector helped the child to the directors office on the same hall. The inspector informed the director of what was observed.

Plan of Correction: The staff member who did not ensure the child was with the group will be reprimanded and all staff will be reminded of the site and sound supervision training that they received in September 2018.
